Hi everybody  Haven't been on here for a while but hope you're all keeping well and good. I started Simponi 2 months ago and it has been going down quite well... not much difference over the methotrexate though so we'll see what the bloodwork shows and decide to continue or not. My question for you guys is regarding vaccines and being on a biologic. I have read in the patient info that we should not have a 'live' vaccine.. does anyone know if the whooping cough vaccine is live or not... I'm due to have it this Friday and been trying to research it but no luck yet. Thanks for your wisdoms. Cheers.  Rob

Hi Rob, I'm guessing you're getting a Tdap vaccine? That vaccine contains pertussis vaccine (AKA whooping cough), as well as diphtheria and tetanus. Here, it just gets rolled into one vaccine, usually. From what I can tell, the Tdap is NOT a live vaccine, but I'm not sure which vaccine you're getting or how they combine them in Australia, so I really think you need to contact some professionals. So, you should probably play it safe on this one - perhaps phone the clinic where you're getting the vaccine, tell them that you're on a biologic and ask them to find out for sure that the vaccine that you're getting is not a live vaccine. I'd also contact the doctor that prescribed the Simponi and ask him/her just to be safe as well. If you can't get a firm answer before Friday, I would delay getting the vaccine until you can find out if it's safe for you. Better safe than sorry. But I'm quite sure the clinic administering the vaccine are used to these questions and will have some answers for you. Good luck with it! And good luck with the Simponi!  Here's a link with a ton of info: http://www.immunize.org/askexperts/experts_per.asp Here's a bit from that page describing the different pertussis vaccines over this way: There are two basic products that can be used in children younger than age 7 years (DTaP and DT) and two that can be used in older children and adults (Td and Tdap). Some people get confused between DTaP and Tdap and others get confused between DT and Td. Here's a hint to help you remember. The pediatric formulations usually have 3-5 times as much of the diphtheria component than what is in the adult formulation. This is indicated by an upper-case "D" for the pediatric formulation (i.e., DTaP, DT) and a lower case "d" for the adult formulation (Tdap, Td). The amount of tetanus toxoid in each of the products is equivalent, so it remains an upper-case "T." .... Tdap has two contraindications and four precautions. The contraindications are (1) anaphylactic reaction to a prior dose of the vaccine or any of its components and (2) encephalopathy within 7 days of a previous dose of DTaP or DTP; in this case, give Td instead of Tdap. The precautions are (1) moderate or severe acute illness; (2) history of an arthus-type hypersensitivity reaction following a previous dose of a tetanus toxoid-containing vaccine; (3) Guillain-Barrè syndrome (GBS) 6 weeks or sooner after a previous dose of tetanus toxoid-containing vaccine; and (4) progressive or unstable neurologic disorder, uncontrolled seizures or progressive encephalopathy until a treatment regimen has been established and the condition has stabilized. If you don't HAVE to have it, I'd put it off. If you have to, ask to see the vial the vaccine comes in...it should say if it is live-virus or not. That way, you are sure no one is giving you the wrong answer. Killed virus is okay (like flu shots) and, as you found, 'live virus' is contradicted.
If you have to have it and it's live virus, your rheumy should be able to give you a letter stating why you can't have it. I had to do that at work last month, because I was supposed to get an MMR, which is live. Rheumy gave me a letter, which employee health put in my file. I still had to sign a declination form. You should be able to refuse it and sign a form. Australia is probably not much different than USA on this. Good luck.
