If you want to use this QR code (Quick Response code) just save the image and paste it where you want. You can even print it and use it that way. Coffee cups, T-Shirts etc would all be good for the QR code.

Hello again,
Sorry for not responding to all these posts earlier. I've read all the responses and they are all helpful. Its reallly appreciated.
I have my rheumy appointment on 8th June. Having read the answers here I'm a little less nervous.
