Thanks everyone for the replies! I found the the address of the abstract I mentioned: http://www.ncbi.nlm.nih.gov/pubmed/7837155It does not explain what severe disease means, so - as John said - what one has could already be that. It also does not mention any treatment for the patients involved, so the picture could be different with effective treatment (the article is from 1994).

i guess i just hate the subjectiveness of the words "severe" and "mild":
for years, the GP, rheumatologists, radiologists referred to what i was going through as "mild".
but when i saw a specialist in the spondyloarthropathies, he said that he thought the main reason i went undiagnosed for so long was because of how severe (and uncommon in its severity) my enthesitis was.
i've had so many contradictory opinions when it comes to the issue of severity that the words don't mean much to me anymore.
to me "severity" means how much pain am i in? and how disfunctional am i?
i think its best to just deal with what is gong on at the moment. i do think the future is just too hard to predict.
but that's just my opinion.

LDN/zanaflex/flector patches over SI/ice vits C, D. probiotics. hyaluronic acid. CoQ, Mg, Ca, K. chiro walk, bike no dairy (casein sensitivity), limited eggs, limited yeast (bread)

I hope it's not true or I'm in trouble! Other than my shoulders my hips and pelvis have been a struggle for me. I have had major flare ups where my pain lasted 18mos. in my hips.  Since I started the NSD I have been able to control the severity of my flare ups and pain.
