I have been in pretty pain-free shape for a few years and have just flared up big stylie since Nov last year. Previous flare-up have been eliminated with LSD and exercise. This time around I am struggling to improve at all.

This last three weeks have thrown everything at it
- No Starch (gone that extra mile and cut out beer)
- Using a TSE/TENS electronic pain relief device
- Actually doing my daily mobility exercises !
- 4*week exercise
- Weekly massage, since back muscles have been going into spasm

To relieve pain, I have been taking 400mg Ibuprofen 4*daily. Still having a bad time so went to docs today who has upped to 600mg and added paracetamol with codeine.

Looking at some recent posts about NSAIDS - this could be counter productive. Is this correct ?

Finally, I asked the Doc for antibiotics to kill the Klebsiella, so was prescribed cofalexin. Anyone experience of this ?

DragonSlayer - you look like you know your onions ! Any feedback appreciated, particularly a duffers guide to NSD and effect of NSAID - I am not familiar with this latest thinking. And much more science about the gut (lesions ?)

Bilko - you may remember me from the NASS forum. Looks like all that went under and as-assist is not nearly as active so I have migrated here ! Hope all is well with you.

Yes, NSAIDs are counterproductive; we are trying to heal the tract and these do the exact opposite.

WOW! Good move on the antibiotics. This is the right class of agent, but I have never used cofalexin, specifically. My regimen calls for 11 days at 1g/day (I weigh 13 stone and a few pebbles). Usually, by day4 the AS flare 'clears up,' if I have been really good. The problem with antibiotics this strong is that they can also cause a flare by stripping away the gut mucosa and it is important to do anti-candidiasis things after taking these, especially implanting the new bacteria using yoghurt with live active cultures or, if you react to yoghurt, probiotics can accomplish the same thing. I can send to you the full regimen, if you are interested, but the Cipro/Levaquin that I used probably wiped out long-established colonies and that can also cause a flare known as the Jarisch-Herxheimer reaction.

What I am saying is that you might continue having AS problems for some time, also due to the antibiotics--but over the long-term you would be much better, if all else goes right.

One more question - Is Parecetemol/Codeine a NSAID ? (or more importantly, does it damage the tract as Ibuprofen does). Looking at things like Wikipedia it is not entirely clear.

So update after NIGHT 1 ...
I stopped the Ibuprofen last night and first full day on the Antibiotics: A GOOD improvement.
a) Two "remote" problems I have had are an inflamed knuckle and an inflamed shoulder cuff - both significantly better.
b) Also my "benchmark measure" of pain. At about 4am every morning I do the "Pillow Shuffle" - rearranging 4 pillows, and sleeping more upright - at about 45 degree angle - to get more relief. Went thro' to 6am without having to do the "Pillow Shuffle" !!

Cool ! Just the Pelvis, Lower Back & Chest to go now !

No, neither paracetamol nor codeine are NSAIDs and they do not damage the tract, but paracetamol should never be combined with NSAIDs since this specific combination is terrible on our livers. Codeine slows the tract action WAY down, so can exacerbate symptoms that way (geometric multiplication of bacterium X more dwell time in tract = more bacteria crossing mucosa).

It is great that you are doing better, and I hope that it continues and the 'steps back' for pelvis, lower back, and chest are not so many or so severe.

ay, as-assist pretty pointless. I'm as well as ever - but it sounds as though you have been playing fast and loose with the diet? You are lucky to have a doc willing to prescribe antibiotics, how did you persuade him?

Hiyi Bilko,

Great to hear from you. Actually had a bad time for the last 9 months and the NSD actually has not helped one bit - for the first time ever ! BUT get this. It is the first time I have been told by the doctor to go for IBUPROFEN so been hitting that big time for the last 8 months.

All makes sense now if I understand DragonSlayers comments on Leaky Gut Syndrome & NSAIDS.

As for the doctor prescribing antibiotics - I have to say she was brilliant. Listening to the Klebsiella theory, and genuinely interested in the Ebringer info. next time I am in. She spent time looking at her books to find out the best antibiotic to fight Klebsiella, and taking a mucus sample as I have been coughing up a small amount of phlem.

Also been back to her a few times in the last 9 months, so I guess she is running out of options/ideas.

So that leaves me just on the parecetemol & codein - after a very painful weekend (probably withdrawing from Ibuprofen) thinking that I'll need to get worse before getting better (i.e. less pain releif and a few days before the antibiotics kick in).
