My surgery offers everyone who has compromised immune systems both the flu and pneumovac - precisely because they have compromised immune systems and therefore would risk far more severe flu and complications if they did get it. I get offered it not because I am on biologics, but because of the potential of a poorer functioning spleen from coeliac disease, but I'd also ask for it anyway because of my poor ribcage expansion (meaning that when I get even a cold it takes forever to get over it because I can't breathe deeply or cough).
