Do we believe that Klebsiella only produces an immune response outside the gut? In other words, is there evidence that a 'healed gut' can put the disease into remission and allow the consumption of starches?

The immune system operates across a healthy gut, and so can the autoimmune response. But a healthy gut should make remission easier. A damaged gut will worsen the autoimmunity.

There are secondary effects of symptom production due to colonization outside the gut, but the main determining factor in AS is the action across the gut membrane. So in near-total agreement with bilko and will suggest that YES: The only difference between two otherwise identical persons (with identical susceptibilities as in twin concordance studies) is the state of their respective tracts. A fully intact tract can make us much more starch-tolerant, if not totally so.
