Thanks a lot for this info. I wonder if this is more useful for mechanical versus inflammatory pain though. I get a pain flare in the area with Piriformis enthesitis and spasm but no pain with the FABER test or any other SI tests.

Most are for inflammatory pain - typical areas for spondy inflammation areas. There is a video specific for piriformis - check the menus various that will pop up when you click on the videos.

Thanks. Nothing with that test either. I've never had sciatica... yet. I just don't seem to get pain with specific movements. I have been using most of these movements in my daily stretching routine though. I always thought piriformis pain WAS SI joint pain.

here's the way my PTs and physiatrists explained it to me:

when my SI joint is inflamed and the ligaments can't do their job of "holding it together" as they should, the piriformis tries to help out. but its just a little muscle (connects from the SI over to the trochanter) and is used to turn our foot out (or in?) and was never meant to stabilize the SI joint.

so SI joint inflammation makes the piriformis muscle spasm.

what i can tell you from experience. when i have inflammation (pain) in my SI joint, my piriformis spasms. when the inflammation in my SI is dealt with via either nsaid, cortisone to the SI, flector patch over the SI, ice on the SI, the piriformis pain (in the middle of my butt) goes away.

like a lot of our body. like when my neck facet joints become more inflamed, the neck and upper back muscles spasm in response.
