In this issue of Annals of the Rheumatic Diseases, preliminary new criteria for remission in patients with RA—another merit of the new times—are published by Felson et al. 8 This paper is also the result of an ACR-EULAR collaboration, underlining globalisation of the world of rheumatology.

The old remission criteria were like silent films—with disease potentially progressing silently under a cover of remission that allowed substantial disease activity to be present. Abstract
Inflammation of synovium is one of the hallmarks of rheumatoid arthritis (RA). Analysis of synovial tissue has increased our understanding of RA pathogenesis, aided in identifying potential therapeutic targets and has been used in the response and mechanistic evaluation of antirheumatic treatments. In addition, studies are ongoing, aimed at the identification of diagnostic and prognostic biomarkers in the synovium. This paper outlines the currently used procedures for sampling and processing of synovial tissue, and presents a standardised recommendation to support multicentre translational research.

Conclusions Although the number of NLRP3 test requests has doubled over the past 5 years, genetic screening has not contributed to enhanced detection of new index cases each year. There are two possible reasons for this: (i) no clinical prerequisite for genetic diagnosis and (ii) few new large families are now identified (unlike the initial study based on a selection by linkage). Conclusion Chemerin, LCN2 and SAA3 are implicated in chondrocyte pathophysiology, and regulated by other relevant factors that drive inflammatory process such as IL-1&#946;, LPS and adipokines including leptin and adiponectin. WOW back gal - You are busy, sounds fatiguing!

Yes, EULAR have been really 'going it' and then some. Now the premier rheumatologist in France is over in the USA, will be gone for a *year (which means I have to see his second in command when I go to Paris in April) putting together various programmes as well as...dada...bringing rheumies upto speed?

I 'think' it might just be possible that these programmes will lead to better information re RA and the spondy stuff will soon be out there. And when you read the stories, all countries, then one has to realise that this is crucial for patient care and patient confidence.

Just hope in the meantime that France is not neglected re correct training of the physios here. Speaking with my physio yesterday, he confirmed that in Fr. the physio training programme leaves a *lot to be desired. They only 'touch' on thhe spondyarthtides, and come away, qualified, knowing nowt! Sad. My PT is one of only two in the whole of Brittany who actually 'knows' the spondy stuff - he is also interested in torticollis, SPR, RA etc - a really keen chap, keeps himself upto date. Now he was trained in Liege (Belgium) and says that the training is Belgium is much different to that in Fr. Interesting. But bears out everythng that I have heard, learnt, experienced about PT training in Fr. (*very poor trainng!) He is now very involved in putting together an additional training programme for the PT in Fr. - says it is a tough one, but hopes his focus will lead to and be successful in achieving a better programme for, a better qualified and a 'more interested' therapist!

Hi Molly,

Just leaving to catch my plane! It is exhausting but it's rare that I have two meetings back to back. Drove four hours to the first meeting on Wednesday - meetings all afternoon and Thursday morning - then drove home nearly five hours yesterday - took longer due to traffic.

Catch a plane to Vancouver today - meetings this afternoon, evening and Saturday morning - then fly home in the afternoon, get home at 7 pm. Phew! Not good when you have an autoimmune arthritis!

Fortunately, this is rare! And much easier than it was eighteen months ago when I could barely walk.
