hi sheri,

not sure there is a way for us to easily tell, but i know my doctor (GP) can tell by listening to my breathing through a stethoscope. pleurisy will have a different sound to it when we breath; doctor said he'd be able to hear the inflamed tissues rubbing up against one another (i think i have that right).

though maybe i shouldn't assume anything, after the first few times of the doctor ruling out pleurisy and concluding it was rib inflammation, now when i get the same pain in the same location, and placing ice there helps, i assume its rib / sternum pain (usually for me its where the ribs attach to the sternum that is the worst, though also for some reason my lower left floating ribs as well (http://en.wikipedia.org/wiki/Floating_rib)).

one thing that showed inflammation where the ribs attached to the sternum was a nuclear bone scan. i was having a horrible flare there, it was so bad that when i took my walk, i couldn't swing my arms because that would make it so much worse. was going in for the NBS, not for that, but because it had finally been approved by my rheumy after a bad SI and upper back flare. and i was amazed how vividly the inflammation at the sternum showed up. so much so that the radiologist had me turn in several directions and took more scans of that area than they would normally do. told me they "had never seen that before!" hee hee!

so doubt they'd do a nuclear bone scan for the chest pain, its usually done for the SI joint (i think) but it was something that showed the inflammation quite dramatically.

it was also the first time i had a nice visual to go along with my pain. really made me see for the first time the correlation between inflammation and pain.

Thanks Sue,
I never heard of a nuclear bone scan. I have seen or read about a machine that shows inflammation or heat I think, it sounds similar. I would really like to have that done to my chest and lower back. I'll ask my dr about it. Thanks again Sue!

i think its a common test to have done for AS, a good way to look at the SI joint and spine especially. not sure its done as much for other inflammatory arthritises, maybe wendy knows more about this for RA? maybe its very good for showing enthesitis, inflammation at the bone? hadn't really thought much beyond that.

i only know about it from people on here recommending it. its a pretty cool test, about the only results i can easily, visually interpret myself. i can't make head or tail of xrays, MRIs, but NBSs light up "like christmas trees" where there is inflammation, bright white spots on a black background.

not to be confused with a dexa scans for bone density which is different.
